Can you be paid to care for a family member?
Some programs allow payment for certain caregiving arrangements. Whether that includes you depends on the person’s needs, coverage, state, relationship to you, and the rules of the program. Medicare enrollment alone does not establish eligibility for caregiver pay.

Medicaid self-directed services
Some Medicaid programs let eligible participants direct parts of their care, including choosing workers. Whether a relative or spouse can be paid, what services count, and how payment works depend on the state and program.
Ask your state Medicaid agency about available home- and community-based services, self-direction, assessments, waitlists, and permitted caregiver relationships. Do not assume that every program requires the same level of need or allows the same family members.
VA benefits and insurance
VA programs may provide support or benefits for eligible veterans and caregivers. Different programs have different requirements; caregiver support and a payment to a caregiver are not always the same thing. Check with VA about the program that fits the veteran’s circumstances.
If the person has long-term care insurance, ask the insurer whether the policy covers care by relatives and what approval, documentation, or waiting period applies. For an arrangement paid from personal funds, seek qualified legal and tax advice before relying on it.
Questions to bring to the agency
- Which program could apply to this person’s needs and coverage?
- Can a caregiver with my relationship to them be paid? Does living together matter?
- What assessments or approvals are required before services begin?
- How are approved hours, payment, employment responsibilities, and renewals handled?
- Is there a waiting list? What support is available while we wait?
- What respite, training, or counseling is available even if payment is not?
Include the person receiving care
Ask how they want care arranged and who they want involved. A program may require their participation, consent, or an authorized representative. The responsible agency makes eligibility and payment decisions.
