No, we are independently owned. We partner with the hospital to ensure your transition from hospital to home is as smooth and seamless as possible.

No, you won’t need to wear the device all the time. It’s used to take your vitals once a day by opening the app and using the device to accurately record them.

The program typically lasts four weeks after discharge, with monthly check-ins with your nurse following the initial 30 days. The duration can be extended based on your needs and medical necessity.

No, only members of the Community Wellness team have access to your vital signs. However, whenever you visit your doctor, you can show them your vitals history directly through the app.
